Please fill out hand-checks completely. I need a student worker's name and date to refer to if I have a question. I know the form doesn't specify date, but please add it. Also, you need to put the barcode number and the due date on the form. I found the form below mixed up in some of the papers on my desk; unfortunately I don't know how long it's been there.
Anytime you leave a note for Kym, Ginger, or I please put your name and date it. This is just in case we need to clarify something with you.
