Lamination process refresher

I have made one small change to the lamination process and I want to refresh your memory about the form.

If you take a lamination please paperclip the lamination form to the file folder and turn the file folder on its side. See photo below for example. And, don't forget the flag!

Also, remember to fill out the form completely. See the form below and notice that the person who took it failed to fill in their name or mark the payment method.
