Logging vending machine questions

We're aware that the vending machine isn't filled as often as it should be.  In order to help prove our case to the book store, we are going to start logging patron questions about the machine.

The form is being kept at the circulation desk by the presentation room slips.  This sheet is for daily use, so when asked a question write the date on the form and make a tally mark under the appropriate question.  For example, if the patron asked a question about the machine not dispensing their item properly, make a tally under the first gray line.  If they are asking about the machine being out of stock, make a tally under the second line and ask them what item it was they were trying to purchase and note that on the sheet as well.  That way, we might be more aware of what items are selling out the quickest.

This sheet is subject to change as we evaluate the needs, and I'll blog about it again if it does change.

As always, please come to me if you have any questions on how this works.
